Consent to teleconsultation

What a teleconsultation is

A teleconsultation is a medical consultation carried out remotely, by secure video, with Dr Nathalie Gozlan, a family physician holding a licence from the Israeli Ministry of Health.

It allows a diagnosis and advice based on what you describe and show to the camera, as well as on your medical file. It does not allow a physical examination.

Its limits — what you agree to understand

Some situations cannot be handled remotely. The doctor may interrupt the teleconsultation and refer you to an in-person consultation, a home visit or emergency services, if your condition requires it.

In a life-threatening emergency (chest pain, respiratory distress, loss of consciousness…), do not use this service: call 101 immediately.

Identification and file

Israeli regulations require the doctor to verify your identity (name, date of birth, teudat zehut / national ID or passport number) and to document the consultation in your medical file.

You undertake to provide accurate information and to consult for yourself or for a minor of whom you are the parent or legal guardian, present at your side.

Confidentiality

The video consultation is encrypted and is not recorded. Your file is stored encrypted and is accessible only to you and the doctor. See the privacy policy for details.

You are responsible for confidentiality on your side of the screen: set yourself up in a private place.

Your consent

By ticking the box “I consent to the teleconsultation” when booking, you confirm that you have read and understood this information, and that you freely consent to the teleconsultation being carried out under these conditions.

This consent can be withdrawn at any time: you may cancel the consultation or end it, without giving a reason.
